Nurse Practitioner — Candescent Health and Wellness
South Bend, Indiana | 30 hours/week
The Role
We're looking for a Board Certified Nurse Practitioner to see both men and women across our core clinical areas: Primary Care, Hormone optimization, TRT, Gynecologic Wellness, (Peri)Menopause care, and Sexual Wellness.
This role is built around individualized care: every patient's plan should reflect their actual history, present concerns, overall goals, and body. Our team does not take a one-size-fits-all protocol. Above all, we want every patient to walk out feeling seen, heard, and valued.What You'll Do
Manage a patient panel spanning primary care, health-span prolongation, hormone optimization, TRT, perimenopause/menopause, and sexual wellness for both men and women
Take the time to build individualized relationships and treatment plans based on each patient's full clinical picture
Order and interpret labs relevant to hormone and metabolic health
Grow into aesthetic procedures (tox, filler, Emsculpt Neo, Emsella, EmFemme 360) with training and mentorship provided
Collaborate closely with our clinical and administrative team — we're small enough that everyone's input matters
Help shape protocols and patient experience as we grow
What We're Looking For
Active NP license in Indiana
2 years of experience preferred — new grads are welcome to apply, we'll train the right candidate
Genuine interest in health optimization, hormone health, sexual wellness, and/or aesthetic medicine. We’re not here for the gimmicks, our practice is built on the symbiosis of evidence and preventative care.

Medical Assistant — Candescent Health and Wellness

The Role
We're looking to fill multiple roles for part-time Medical Assistants to support our clinical and administrative team. This is a hands-on role in a small practice. Your responsibilities will include rooming patients, assisting with procedures, managing front-desk and EHR tasks, and helping create a great patient experience from check-in to checkout. No two days look exactly the same, and we like it that way.
What You'll Do
Room patients and prepare charts ahead of provider visits
Assist with in-office procedures — injections, IV therapy setup, suture removal, aesthetic treatments (tox, filler, Emsculpt Neo, Emsella, EmFemme 360)
Perform phlebotomy and specimen handling
Manage scheduling, insurance verification, and patient communication
Maintain accurate documentation
Help keep exam rooms and supplies stocked and organized
Support marketing and patient experience efforts as needed.

What We're Looking For
1–2 years of Medical Assistant experience preferred
Comfortable with phlebotomy and injections (IM/subQ)
Experience with electronic charting
Strong communication skills. negotiable skill of compassionately discussing sensitive topics (hormone health, weight, sexual health, aesthetics) with patients
A team-player mindset; flexible and willing to wear multiple hats
Genuine interest in adult health wellness, hormone health, and/or aesthetic medicine

The Role
We're looking for a Client Experience Lead to be the face and voice of Candescent. You'll be the first person patients talk to and the last person they see on their way out… and everything in between. This role sets the tone for how patients feel about our practice, we need someone who leads with warmth, stays calm under pressure, and genuinely enjoys taking care of people.
What You'll Do
Answer phones & manage scheduling with a warm, professional first impression
Check patients in and out, keeping the front desk running smoothly
Address patient needs and concerns as they arise, with a compassionate, patient-first approach
Serve as the go-to problem-solver when something doesn't go as planned, whether its a billing question, a scheduling conflict, a frustrated patient or anything else that comes your way.
Coordinate closely with our clinical team to keep the day running on time and communication flowing with patients and between providers
Help maintain a welcoming, organized front-of-house environment
Support administrative & marketing tasks as needed… we're a small team and everyone pitches in
What We're Looking For
Experience in a front-desk, client-facing, or patient coordinator role (medical/aesthetic practice experience a plus, not required)
A calm, compassionate approach to handling patient concerns. The ability to de-escalate a hard moment without losing warmth
Strong organizational skills and comfort managing a busy schedule and multiple moving pieces
Excellent communication skills, written and verbal
Comfort with EHR/scheduling systems (athenaOne experience a plus)
A genuine desire to make every patient feel seen, heard, and valued
